feat(hygiene): nag about full memories

When memories become full the hygiene routine will open a nag buffer
about them (once). This is enabled by default and controlled by
`partial-recall-warn-on-full`.

This also adds `partial-recall-flush` to make flushing configurable.

This also adds new indirection function
`partial-recall-memory--unique` so that functions from other modules
don't need to access slots directly (which is mostly relevant for
tests).
