Docs: Fixed and remove broken links to HMC books

Details:

* Removed entries in the Bibliography for WS-API book versions 2.11/2.12,
  since they no longer seem to be publicly available.

* Fixed broken links to the WS-API book versions >=2.13, since IBM changed
  the links. The links for versions 2.13/2.14 use an older (but working)
  link that will alsso be migrated to the new module-based links.

* Removed entries in the Bibliography for what was called "HMC Operations
  Guide", and removed the (one) reference to it. This book no longer exists
  and has been replaced with a PDF version of the HMC Help System. Since
  it is too cumbersome to link to specific chapters in a PDF, the
  reference to it in the introduction section of the docs has been removed,
  and it was not really necessary anyway.
