Fix build if permalinkPrefix is not '/'. Fixes #6

Ignore permalinkPrefix altogether in start mode to keep the preview working as
expected. Now the host server can't be expecting a working build if writ is run
in start mode. This needs communication.

Also:
- Add outputPath field to post, subpage and category models
- Add tests covering cases of permalinkPrefix being /blog, in both build and
  start mode
- Extract test utils
