Merge 0.45.3 into stable/0.46 (#11698)

### Summary

This brings in all the changes from the 0.45.3 release into the 0.46
series. This should be the last up-merge, since the 0.45.x series is now
closed.

The only conflict was in `qiskit/__init__.py`, where this commit unifies
the "warn if 'qiskit' isn't installed" and "error if Qiskit 1.0 is
installed" behaviours into the same handling.
