Refactor summarize_results

The simple summary that is printed for MypyUpgradeResults is now
implemented as MypyUpgradeResults.__str__ to localize the logic related
to MypyUpgradeResults.

The detailed summary printed for each set of errors (silenced, failures,
and ignored) is now encapsulated in a function to eliminate repetition.
