belayd: plugins: Add a plugins directory for causes/effect to be registered at runtime

Add a src/plugins/causes directory to hold causes that a user can
register at runtime.  These causes are not built into belayd, and must
be added via belayd_register_cause().

Also add a simple test that exercises the automake and include logic
required to do this.
