Implement support for document segmentation

Subclasses of PDFAnalyzer can now override document() to return a list
of 3-tuples describing appendices or other parts of a PDF document
that shouldn't be analyzed/parsed according to main rules (since they
can have completely different metrics etc). Any document part that's
not tagged with 'main' will be output "verbatim" in the resulting
XHTML, meaning that each text string stands alone (not grouped into
paragraphs etc) with explicit placement information. Rendering this as
a HTML page will yield a very PDF-like page (but not
reflowable/responsive etc).
