* Remove Name field from Entity and ViewModel constructor as it should always be an empty string which is never displayed anywhere

* Added BusinessLogic folder structure for AdaptivityContent commands

* Added CreateAdaptivityElementForm and EditAdaptivityElementForm

* Consolidated CreateAdaptivityElementForm.razor into CreateElementForm.razor

* Adapted EditElementForm to AdaptivityElement

* Added tests for CreateElementForm and EditElementForm for adaptivityElementMode Parameter
