[#13] Improve ES trials mapping

With this changes, attributes like "id" and "locations.roles" aren't analyzed
by ES, leaving them as is. Also, the "locations.name" attribute is copied to a
"location" attribute, allowing the user to create queries like:

    /search?q=heart+disease+location:Brazil

Instead of the current:

    /search?q=heart+disease+location.attributes.name:Brazil
