refactor: disconnect sessions concurrently

Previously, we invoked Disconnect calls one by one even on server shutdown. Now we perform them in parallel (which makes more sense, since the goal is to notify RPC of all closed connections).

You can restore the previous behaviour by setting --shutdown_pool_size=1
