Added assertion flags & runtime I/O exceptions

- Assertion flags will throw exceptions as soon as possible, which
  should be convenient & more secure for unchecked exceptions
- Runtime exceptions will be thrown when basic IO operations fail
  despite preceding checks

At this moment warnings are silenced and replaced with custom messages,
but it might be a good idea to capture them with `error_get_last()`,
and pass into `IOException` as previous one (3rd constructor argument).
