fix: security issues caused by outdated dependencies

- remove coveralls from gemspec
- add versions for coveralls and mime-types to Gemfile
- update ruby-saml to 1.1.2
- update rack-test to 0.6.3
- remove Gemfile.lock (It seems to be bad practice to checkin the
  Gemfile.lock when working on a gem.)
