refactor debug lib; version 0.9.0

- Rename Exception to Debug library. Fix all directories and imports.
- Rename Libraries to use a dot to separate the interface suffix, as in `forandar.cli` `forandar.web` and `forandar.core`. Leave the underscore to separate between compound words like `virtual_machine`
- Move InputQueueCli class to `src/cli/input.dart`.
- Move InputQueueWeb class to `src/web/input.dart`.
- Export all relevant libraries from the entry points.
- Add debug tests under `tests/debug/`.
- Add forandar_debug executable.
- Add new file `tool/test.sh`.
- Update and fix comments.
- Fix tool scripts.
- Update to version 0.9.0.
