feat(ngx_lua) support for all non-cosocket contexts

- refactor new_socket() (hot code path) to be more performant
- only require LuaSocket if lua-cassandra was required in a context
  without cosocket support (typically init)
- new cosocket proxy mt for LuaSocket (can be used to monkeypatch
  unsupported functions)
- correctly check for phases that support cosockets
- new t/context.t integration test suite
