More HTML parser testing, with bugfixes.

- Now keeping track of the <head> element pointer and resetting
  insertion mode correctly in <html> elements.
- Not inserting implicit <body> tag for a fragment consisting of a
  <head> element. This is a deliberate deviation from comformance.
  Similarly, not inserting <body> tags for fragments consisting of
  elements that belong in <head>.
- Not inserting implicit <head> tag for fragments beginning with a
  <frameset> or <body> element. This is again a deviation from
  conformance, in order to support intuitive fragment parsing.
- Correct handling of <frame> fragments.
- Better handling of <frameset> in <body>. When the <body> tag is
  implicit and the parser has to switch to <frameset>, it has no good
  way of removing signals already emitted while parsing <body>.
- Correct handling of <iframe>, <noembed> elements.
- Correct handling of generic end tags in <body>.
