fix: synchronous fetch on initialization (#196)

* Synchronous fetch on initialization should fail if upstream is not available. This is the behavior in most SDKs but not this one. This is potentially a breaking change if you're not expecting it to fail when enabling the synchronousFetchOnInitialization flag
