feat(auth) AuthProvider pattern similar to node.js

Implement a more versatile authentication pattern: since issues have
been reported the driver cannot be used to authenticate with Instaclustr
(because it does not use the C* plain password authentication class),
this follows the Node.js driver pattern: an authenticator instance must
be given to the session and will be used against clusters asking for
authentication, regardless of the class given in the authentication
challenge response of the binary protocol.
