Change result from http_get_provider

Prior to this change, http_get_provider made a JSON web token for you.
It contained the provider’s user ID as the user_id key. This isn’t
suitable for most purposes, since the provider user ID is likely not the
application’s user ID.

Now http_get_provider returns a provider_user_name and provider_user_id
and does not set a final JSON web token. It still sets an intermediate
web token during the OAuth flow, but not at completion. This change
should not affect the first step of the OAuth flow.

The docs and test coverage have been updated accordingly.
