Fail tests where adapter can't be initialized

PHPUnit doesn't exist with failure status code for incomplete tests.
Also, I'm incorrectly using "incomplete" here - it's meant to surface
tests that still need to be expanded, not unsatisfied preconditions
(missing adapter)
