Merge pull request #220 from matthew-brett/arraywriter-refactor

MRG+API: extend / minor break of API allows explicit scaling

Merge machinery to make it possible do explicit set of image scaling for Analyze type images including NIFTI.

I did my best to keep the API as similar as possible, and I don't think you will
see changes to common nibabel behavior as a result of this merge, but I'd very much
like to hear if anyone does see something surprising.

One side-effect of the changes is that we no longer check and fix odd scaling
when we load a header from file, because we are always going to reset scaling
to NaN when creating images, and the log checks also made it harder to reason
about what was happening with the scaling.
