Rate accumulated usage

Rate accumulated usage when accumulating usage quantities at usage
accumulator. Standardize accumulated usage windows by replacing
accumulated_usage.quantity with accumulated_usage.windows. Avoid
mix and matching objects and integers by initializing windows with
default null objects.

Update Aggregator to use the modified accumulated usage when aggregating
the accumulated usage.

Update CF usage accumulator application configurations.

Update unit and integration tests to reflect the design change in
accumulated usage.

Implements #159.
