Keep long pipe lines whole, name the cause of a failed command in its fault, and route package functions through CommandConfig

- ExecInPipe reads whole lines, so a line past 4096 bytes comes back in one piece; a read fault now gets returned instead of a panic
- When a command fails, the fault message now carries what the command printed as its reason, and errors. As keeps finding the exit code
- MatchPipe sees one line at a time across both pipes; the comment that said a match makes a failed command pass is fixed, as the exit code decides
- Exec, ExecInPath, ExecInEnvs and ExecXshRun go through CommandConfig with the same output, and a bad config logs its fault once instead of twice
- WithExpectExit works on a bare CommandConfig{} too, and ExecWith accepts an absent prepare func
- Rename the utils function ExceptsCode to ResolveExitCode
- go.mod moves to Go 1.25 to use wg.Go; the CI matrix runs 1.25 to 1.27 with the new action versions
- README: badges come from the template, a section on streamed output, a note on how args get quoted, a printf pipe example that works, and the missing API entries
- Tests check results and read as usage samples, each exported function now has a test, and each demo ignores its build output
