Add dal-simplifier agent; reframe two sweeps as out-of-band

- New dal-simplifier agent: a find-and-recommend sweep that enforces the
  "No Duplication" and "Comment Style" rules in .claude/rules/code-style.md.
  It reads already-implemented code (a diff, a PR, or a whole module) and ranks
  duplication, near-duplicate types, dead code, verbose constructs, and
  oversized comments. Sibling lens to dal-reviewer; edits code only on
  explicit opt-in.
- Reposition dal-performancer and dal-simplifier as out-of-band quality sweeps
  (separate context, often background, on demand), with dal-reviewer as the
  sole blocking in-loop gate. They were previously drawn as in-loop parallel
  steps; in practice they run separately and should not gate every iteration.
- README: workflow diagram split into a main in-band flow ending at the merged
  PR plus a dashed out-of-band lane; roster row added for the simplifier; team
  working agreements updated to cover the simplifier's apply mode.
