Address review of the DB persistence layer

- Delete unused get_engine / session_maker / reset_engine_cache / _engine_cache:
  DbStore builds its own engine and the store singleton already holds it, so the
  process-wide cache was dead weight referenced only by the test fixture.
- Make DbStore's url required so a bare DbStore() can't silently fall back to the
  default file; the factory (_build_store) now resolves DAL_WEB_DB_URL/default.
- Add test_update_valuation_persists_across_reopen: the async pricer writes
  results back solely via update_valuation, so its patch must survive a reopen.
- Document the portfolio_trade position invariant in add_trade_to_portfolio.
