fix: make API key optional for custom providers

skip Authorization header when API key is empty to support
custom providers that don't require authentication (local LLMs,
development servers, etc.)

previously, empty API key would send 'Authorization: Bearer' header
which could be rejected by some servers. now conditionally add
the header only when API key is non-empty.

add tests for both OpenAI and CustomOpenAI providers to verify
header behavior with and without API keys for both chat completions
and responses API endpoints
