feat: add --force flag and improve file exclusion handling (#26)

This commit addresses issues with file pattern matching and exclusions,
particularly for files in build directories that were being incorrectly
ignored.

Changes:
- Add --force flag to skip all exclusion patterns (gitignore, common patterns)
- Remove **/build/** from common ignore patterns to allow build configs
- Auto-enable force mode for concrete file paths (no wildcards)
- Improve error messages to explain why files weren't found and suggest --force
- Add comprehensive tests for all new functionality
- Update README.md with force flag documentation

The --force flag allows users to include files that would normally be excluded,
while automatic force mode for concrete paths ensures that explicitly specified
files are always loaded regardless of exclusion rules.

Related to file pattern matching improvements and user experience.
