3.0221: Fix handling of unicode noncharacters

Only a few unicode characters are noncharacters:
U+FDD0..U+FDEF, U+FFFE, U+FFFF
Add a nonchar/utf8 warning, replace only in non-relaxed mode.
Fixes #74 and #73.
