Format images as srcsets when exporting

**Before**
All the images were exported independently no matter if they were
actually used.
Also, the exported manifests included the image data instead of
srcsets with paths to spec-ed image files.
Finally, images were all stored flat in an `images` folder without
grouping them.

**Now**
Images are exported when exporting pages which means that images
not used are not exported.
Also, the exported manifests now include srcsets instead of image data.
Finally, images are now grouped by 'page' (e.g. `images/hernia`)
