* lint happiness
* consolidated 3x checks info client_disconnected()
* wrap long lines
* refactored log_run_item() out of ginormous run_next_hook()
* replaced utils.in_array() with faster switch/case in new fn is_deny_retval()
* invert logic in two loops in run_next_hook(): less indention and early exits
  make it easier to extend the conditionals (coming soon to a PR near you).
