* [Claude] Add PHP 8.5 to CI test matrix

* ci: Fix Coveralls individual status checks failing

- Replace php-coveralls with official coverallsapp/github-action@v2
- The official action properly handles parallel builds without creating
  individual status checks for each job
- Update actions/checkout and actions/cache to v4
- Update coverallsapp/github-action from @master to @v2

This fixes the issue where each DB driver test created a failing status
check with partial coverage (14-55%), even though the aggregated
coverage is 100%.
