Undoes some v1->v2 transitional code (#242)

* Removes transitional v1-v2 logic now that the platform is 100% v2.

* Removes the unsed import of 're'.

* Updates prometheus_status unit test to test against v2 names, as all of them now are.

* Removes the concept of 'flat' names from fqdn_rewrite now that all names are flat by default, and updates the unit tests to reflect this.

* Removes the word 'Plaintext' from test names in test_fqdn_rewrite.py.
