fix(build): restore callable CJS entry via add-module-exports

0.14.0's published entry was directly callable via require(); the babel
CJS build emitted only exports.default, breaking plain require('superpipe')
and the interop smoke test caught it pre-publish. add-module-exports
assigns module.exports when the module has a single default export.
