Upgrade to modmath 0.4, const-num-traits 0.1.1; drop fixed-bigint (#28)

upgrade to fixed-bigint 0.4, modmath 0.4, add const-num-traits 0.1

Ct / Nct / Personality moved from fixed-bigint to const-num-traits in
the 0.4 generation; re-export was dropped, so all 12 source files that
import those types now reference const_num_traits directly.
