Require wcwidth>=0.9.1 and use terminal corrections

wcwidth 0.9.1 ships the libwcwidth C extension, and since 0.8.0 adds
terminal-specific corrections for grapheme widths.

Measure text with wcwidth.width(..., term_program=True). font.py and
text_layout.py now go through calc_width, which drops extra wcwidth
imports.

The test suite unsets TERM and TERM_PROGRAM so expected widths do not
depend on the environment.
