docs: document embedded migrations support

- record the embed FS implementation status and the deltas against the original
  design (runtime level fsys instead of command globals, added LoadFS, create
  stays disk only) in the embed fs design doc
- mark stage 4 (embed FS) as done in the runtime architecture design doc
- add an embed.FS library example plus SetFS/BindFS notes to both READMEs, and
  fix the example imports (miglite.Config, pkg/command)
