docs: add language-neutral specification in docs/spec/

13 Markdown documents covering the full PrestoPlot system in
hexagonal/Clean Code/DDD style:

- Domain model (Grammar aggregate, Rule subtypes, Database entity,
  value objects, domain services)
- Ports and adapters (GenerationCommandPort, StoragePort,
  TemplateEnginePort, RandomPort, all driving/driven adapters)
- Grammar file format (normative YAML reference)
- Application services (5 use cases with pre/post-conditions)
- Selection modes (reuse, pick, ratchet, markov, list -- with exact
  behavioral contracts and lifecycle specs)
- Seed and context (MD5 algorithm, scoping protocol, determinism)
- Markov chain (training and generation algorithms, normative vectors)
- Template engines (ftemplate and jinja2 port contracts)
- Storage adapters (FileStorage normative; caching adapters informative)

- Test specification (60 GWT test cases, normative/informative tagged)
- Appendix with full annotated normative grammar files
