feat: release v1.0.0 — stable API, fixed exports, reduced bundle

Breaking changes:

- Stream utilities moved from 'pure-md5' to 'pure-md5/stream'

- WebCryptoBackend renamed to BrowserBackend

- IE11Backend and 'pure-md5/adapters/ie11' removed

- FallbackManager, MetricsCollector, robustHash removed from public API

- CJS require() subpath exports now resolve to .cjs files

Fixes:

- Fixed exports map: require paths pointed to ESM files

- Fixed hashBinary corrupting non-UTF-8 byte data via TextDecoder

- Added stream/index build entry (pure-md5/stream now resolves)

Improvements:

- Main bundle: 18KB -> 7KB minified, no fs/stream dependencies

- Removed dead code (md5blk, stream/light)

- Expanded npm keywords for discoverability

- Updated README with accurate bundle sizes and import paths

- Added 30 tests for binary data hashing across all backends
