Apply review clarifications to the walkthrough

Give the end-to-end example its own README section, complete the
sender narrative through signing and broadcast, separate the two
privacy mechanisms correctly (OHTTP encapsulation hides content from
the directory, the relay hides client identity; the directory can
still link one session's requests through the shared mailbox), drop
an unclear sentence, and note that output identification also
guarantees at least one output pays the receiver.
