Allow user to create user satisfaction data set

When users try to add a user satisfaction module to their dashboard
they are redirected to an error page if the user satisfaction data
set doesn't already exist.

Changed the behaviour to allow the module creation process to do a
get or create on the data set if a 'done' page for the service has
been set.

Not sure if this change is appropriate.  Will check how user
satisfaction modules are supposed to be set-up.

This problem does not affect admin users which is odd.
