Split components and merge coverage object.

 * Split out analyzer and formatter to npm modules.
 * Use proper CLI tool for generating lcov data.
 * Don't use separate `counters` property for coverage.
 * Update documentation.

This is a fairly big update that significantly streamlines the internal coverage API and makes it much easier to consume coverage data from outside. The only data format that gets output is the raw JSON coverage object – lcov data can be generated from `adana-cli` with `adana-format-lcov`.
