Flask Migrations

This assumes use of `.env` for local configuration.

You can use `honcho` or `heroku local` to run commands automatically
loading `.env`.

Example to run dev server:
`heroku local` or `heroku local:run python manage.py run server`

Example to create new migration:
`heroku local:run python manage.py db migrate`

Example to apply migrations:
`heroku local:run python manage.py db upgrade`

`app.cfg` loading has been removed, but we could leave it if there is a
reason I’m not thinking of to keep it around in addition to the `.env`
loading.
