* fix: serve keys that were already waiting before newcomers

A holder that queued again was appended at the tail and treated as the rotation cursor. Keys already in line sat in front of that cursor, so every later arrival was granted first and those waiters never ran while new keys kept arriving.

* fix: don't let a later waiter inherit a sealed round

The round was stamped on the key, so every new waiter on a key that had already joined cut in front of keys already waiting. A key that never drained held the round shut.
