feat: let `use Bandera` ship lowest-precedence default settings

An application embedded in other applications (a dependency whose
LiveViews a host mounts) could only get its instance's storage and
notification settings from application env, which a host never loads
from a dependency — so every host had to mirror them, and a drifted or
missing mirror silently fell back to in-memory storage.

`use Bandera, otp_app: :my_lib, defaults: [...]` now sets the instance's
lowest-precedence layer: defaults < application env < explicit child
options, merged one level deep. A host overrides only what it must (e.g.
its test store). `__bandera_defaults__/0` exposes the shipped defaults.
