Documentation updates

* Add the README to the generated docs as the landing page

* Remove the doc directory from the repo (these should be generated by
hex automatically and can be easily generated by a user)

* KafkaEx.Config reads config/config.exs and inserts it into a code
block - this was an attempt at a solution for not duplicating
documentation of configuration options.

* Rewrite parts of the first couple sections of the README
