Rename aliased InteropMiddlewareInterface to ServerMiddlewareInterface

Per a comment by @xtreamwayz, there was inconsistency between usage of
the alias `InteropMiddlewareInterface`  in the `src/` directory, and
`ServerMiddlewareInterface` in the `test/` directory. Since the ultimate
goal is for http-interop/http-middleware to be accepted into php-fig,
the latter verbiage will be more correct long-term. This patch makes
that update.
