Change work generator to insert after last created work, if any

Should fix many problems with having a second concern in CI.

Note: this doesn't *actually* resolve the problem of
`primary_work_type`, but it mitigates it by making the behavior less
surprising.  I.E., generating a new concern shouldn't change the
"primary" work type.
