Add ignoreSelfMiddleware and options to configure (#64)

* Add ignoreSelfMiddleware and options to configure

- Adds ignoreSelfMiddleware() as default middleware, but this will only work if `meta.app_bot_id` is set by the context. You must use `slapp-context-beepboop` >= 1.3.0. 
- Added `opts.ignoreSelf` to configure this
- Makes ignoreBotsMiddleware() optional by default
- Added `opts.ignoreBots` to configure this, false by default
- Added tests around new middleware
- Updated README
- fixes #63
