Fix potential `printf` bugs everywhere

After fixing the problems that caused the build breakage in #98, I
audited all other `printf` calls and added tests where possible to
trigger similar problems and verify their fixes. Specifically, all
`printf` statements contain `--` and `'%s'` where applicable.

Contains other small additions and refactors as well:

- adding `test_filter` to some `setup` functions
- cleaning up a stray `echo` in `printf_with_error`
