add channel-specific slack notifications (#1145)

* add hint to notification fields label on App form
* add the ability to select what channel slack notifications will be send to
* enhance slack_service_spec.rb to test slack notifications with and without channel
* add validation specs to NotificationServices::SlackService
* fabricate the correct room_id for :slack_notification_service fabricator
* only display em tag for service_notification_fields if field[:info] is present
