Apply inline and normal options in single pass

Inline option module now doesn't apply the options immediately
in check step. Instead it saves relevant options and push/pop
directives. This information becomes part of check() return value,
and is cached fully.

During join/filter step for each issue relevant inline options
are calculated and applied on top of normal options.

This fixes issues with inline options unable to ensure that
a warning is not filtered, e.g. by setting a restrictive std -
second pass using normal options would filter it anyway.
